Description: 09-26-2006- i conducted an inspection of 180 pebble trl. and observed a large mound of wood chips and yard debris on the front yard of the residence. also the grass was too tall in violation of the property maintenance code. located in the carport, visible from the street was garbage and rubbish strewn about the on the ground. attempts were made to contact the resident and no one answered the door. i posted the dwelling with a warning sticker. 10-03-2006 - i reinpected the property and observed no change or improvements from the 09-26-06 inspection. i noticed all three of the warning stickers i placed on 09-26-06 were removed. i re-posted three warning stickers on the property (front door, side door and mail box post.) 1500 hr - i called 770-475-4108, which is the listed phone number for jane bates, the owner of the property. i left a message for ms. bates on her voice mail. 10-03-2006 - i submitted two municipal ordinance violations (mov) to ms. janes bates for violations of the cpmc 302.4 and 307.1. the mov's were sent alongwith another letter to ms. bates via regular, certified mail (usps) and fedex. the mov's and the stipulated that ms. bates had until 10-18-2006 to maintain her property to code compliance to avoid a formal summons to appear. 10-04-2006/0830 hr- i called ms. bates phone number and left a message for ms. bates on her voice mail. 10-06-09- i called ms. bates phone number and left a message for ms. bates on her voice mail. 10-09-06 -i called ms. bates phone number and left a message for ms. bates on her voice mail. 10-18-06 - the property was inspected and the noted violations were corrected.

Description: 10/02/06 - an inspection of 315 karen dr. revealed a large trailer parked in the back yard of the property filled with rubbish and other construction materials. the trailer could be seen from the street. at that time i contacted the residents and spoke with mrs. ana viezcas, who indicated she is married to the owner of the property, mr. oscar hernandez. i advised mrs. viezcas to correct the violation. 10/09/2006 - an inspection revealed no change to the property and it was still in violation. i spoke with the homeowner, mr oscar hernandez and he stated he would remove the rubbish this week. the property was posted with warning stickers. 10/16/2006 - an inspection revealed no change to the violation status and mrs. viezcas was spoken to about this. a warning notice was mailed to the residents at 315 karen dr. via regular and certified usps. 10/23/2006 - another inspection of 315 karen dr. revealed no change to the property and still in violation. 10/24/2006 - a municipal ordinance violation (mov) was submitted and hand delivered to mrs viezcas at 315 karen dr for violations of cpmc 307.1. accumulation of rubbish. the homeowner was given until 10/30/2006 to comr into compliance. 10/30/2006 - the property was reinspected and the noted violations were corrected and in compliance.

Description: 10/19/2006 - i inspected the property at 400 thompson st. and observed the grass/weeds were too tall, large amounts of rubbish was strewn about the property. the property was posted and attempts were made to locate the owners of this abandoned property. a warning letter was mailed out to mr. douglas avren, the listed owner of 400 thompson st. via the fulton county tax assesors office website. 10/23/2006 - i met with mr. avren who indicated he no longer is the owner of 400 thompson st. mr avren provided documents showing that he sold the property to bruce williams properties llc, which is a subsidiary of thomas enterprises, inc. which is the developer of the prospect park development adjacent to 400 thompson st. 10/24/2006 - i contacted mr. thomas tropea, vice president of property management for thomas enterprises, inc. i explained the nature of my investigation and mr. tropea stated he would remove all of the debris from the property to satisfy any neighborhood or city complaints of code violations. 10/27/2006 - i reinspected the property and found it to be in compliance.

Description: 8 feb 07 - received call from mr. tim braue - 770-475-2402 - about vehicles and parking on unprepared surface at 8015 canter ln. observed address from the street, but did not note a violation. i did note an area of the yard to the right of the driveway that appeared to be used as a parking area. 14 mar 07 - noted vehicle parked on an unprepared surface in the yard. i was unable to speak to anyone at the address. made photograph. 4 apr 07 - noted trailer with boats and vehicles parked on unprepared surface. made photograph. 20 apr 07 - attempted to speak to the owners, no answer at the door. 30 apr 07 - attempted to speak to the owners, no answer at the door. 1 may 07 - spoke to the resident at his property about previous observations of boat parking and vehicles parked on an unprepared surface. i explained the code requirements and discussed alternatives so that the property could remain in complaince. the owner was very cooperative and receptive. 2 may 07 - emailed mr. ward a link to the unified development code, the property maintenance code, and provided contact information for questions about constructing a prepared parking surface.

Description: 12/01/06 - berkshire manor subdivision became a part of the city of alpharetta on december 1, 2006 and is subject to the city of alpharetta unified development code. i observed commercial vehicles (taxis) and inoperative vehicles parked at 3155 berkshire manor dr. prior to the subdivision's annexation. on december 1, 2006, the day berkshire manor subdivision officially became a part of the city of alpharetta, i observed an inoperative vehicle in the driveway of the residence and a taxi (nuevo taxi) parked at the residence. conversation with mr. tom digivoni, the subdivision home owners association president indicated that the residence was possibly being used as a business location and that he had noted repair work preformed on vehicles. tax records list mr. gerardo avila as the owner of the property. 12/27/07 - spoke to resident at the door of the home. she identified herself as esmeralda, the wife of the owner. i spoke to her about the inoperative vehicle in the driveway and informed her that major vehicle repairs were prohibited by the city property maintenance code unless preformed inside an approved structure. i gave her my card with my name and phone number and asked that she have her husband, gerardo avila, contact. 01/05/07 - sent warning notice addressed to mr. avila via regular mail. the warning notice included one violation of the city of alpharetta property maintenance code - section 302.8 - inoperative vehicle, and one violation of the city of alpharetta unified development code - section 2.5.4.b parking commercial vehicles (taxis) in a residential district. 01/19/07 - mr. avila has not contacted me. i continue to observe taxis parked at the residence. i photographed a taxi parked in the driveway of 3155 berkshire manor dr. as evidence that the owner has not taken steps to bring the property into compliance. 01/23/07 - photographed a vehicle that i've observed parked regularly at the property. the vehicle does not have taxi marking nor have i noted evidence that it is used as a taxi. mr. tom digivoni stated he has observed the lights of a taxi meter inside the vehicle at night when he passed. i have not witnessed any evidence that the unmarked vehicle is being used as a taxi. i have observed other vehicles marked as taxis parked at the residence. i prepared hand delivered notice of violation (#2157) to the residence and esmeralda accepted, but declined to sign the notice of violation. the notice of violation was issued because of violation of udc section 2.5.4.b (parking business vehicles i.e. taxis in a residential district in an area visible from the street) and required that the property be brought into compliance no later than 26 january 2007. the notice of violation packet also contained a letter from the alpharetta business license division directing them to cease any business activities at that address. 02/05/07 - re-inspected & made photograph as evidence that at least one taxi continued to park at the residence and that the property owner in not in compliance with the requirements of the notice of violation (#2157) of 23 january 2007. 03/13/07 - re-inspected the property and made photograph as evidence that at least one taxi was parked at the residence, visible from the street, in violation of city of alpharetta unified development code. 04-10-07 - observed that taxis continue to park at the residence. a resident of the subdivision reported that a large number of taxis stop at this address early in the morning (between 2-4 am) and that maintenance and driver shift changes take place. i have not personally witnessed maintenance or driver shift turnover. 04/13/-7 - observed marked taxis at the residence. 04/20/07 - sent final warning notice with photographic evidence, copy of notice of violation # 21 57 of 23 jan 07, & copy of section 2.5.4.b of city udc. warning 05/03/07 - observed that the marked taxi normally parked in the driveway of the residence was parked on the street in front of a neighbor's property. other vehicles not marked as taxis, but appear to be used as taxis (but not observed being used as taxis), were parked in the driveway & on the street. there were a total of seven vehicles parked in the driveway or on the street that appear to be associated with 3155 berkshire manor dr. 05/10/07 - issued summons to appear (#5305) to mr. gerardo avila. summons was sent via federal express - tracking number 792341072534. 05/14/07 - received delivery confirmation that summons to appear (#5305) was delivered and signed for. 5/16/07 - observed marked taxi parked on the street in front of 3155 berkshire manor dr. made photograph as evidence that the property has not been brought into compliance. 5/24/07 - code enforcement board heard the case against mr. avila. 5/24/07 - final judgment & orders of code enforcement board directs mr. avila to ceae and desist violations of city of alpharetta unified development code and that any future violations will result in a $250.00 fine for each and every occurance. 8-6-07 - residents of berkshire manor subdivision report mr. avila continues to park taxis at his house. also reported was maintenance being performed on the vehicles. 8/10/07 - received photos taken by mr. tom digivoni of taxis parked at the property. officer schroeder witnessed taxis parked at the property and spoke to the owner, informing him that he could be fined if he continues parking taxis at the property. 2/19/08 - observed taxi parked at the property at 8:10 pm. i drove through the neighbor on my way home from the berkshire manor subdivision hoa meeting. this supports reports that the taxi is being parked in the neighborhood outside the hours of lunch periods. 2/20/08 - report from the neighborhood indicates the taxi was parked in the neighborhood overnight. 2/29/08 - notification letter prepared for mr. avila reminding him of the provisions of the final judgment & order from the code enforcement board and the possible fines that may be imposed.

Description: 07-01-09 went to 12040 to investigate a complaint that was filed by the owner of 12040 about the property at 12050. complaintant said that 12050 discharges their downspout run off onto the nature area of 12040. went with husband at 12040 to rear yard to see the issue. after looking through the woods for several minutes we found the discharge pipe and it did clearly appear to me that it was on or very close to the property line. i told mr. jacques that i would go speak to the people at 12050 but hat it didn't appear to be causing any real issues but needed to be move back away from the line so that it would discharge on their own property before running down to his(which is the natural slope). i went to the home at 12050 and spoke with the female owner who i showed the problem and she stated she would have her husband call me if he had questions on what to do but other wise he would get it moved. i asked how much time she thought he would need and she didn't know. i asked if 30 days was enough and she stated "plenty".
