Description: Construct 16x38 2 room addition (family room, bedroom w/full bath),crawl space,concrete footer &slab,frame construction w/brick exterior,framed roof system w/vinyl siding on gable endelectricalmechanical and plumbing . . . . 11/22/96 talker to mr baker,the owner.the contractor has requested that his contract be canceled and mr baker has received a complaint form from mhic which he will be filling out and submitting to the commission.on the advice of his attorney he had another contractor,dick mccleary, inspect the property.no work will be continued on the project at this time and i advised mr baker of the requirement for permit changes if a new contractor is obtained.wkv 2/11/97 met with bill wantz in the office.he is the attorney for mr baker.an inspection of the property is scheduled for 2/13/97. 2/13/97 met with bill wantz,attorney,dave cory,3rdparty inspector and me baker the owner and review addition and the following violations were noted. 1.over span on floor joist.joist are 2x10 spf#2 with an span of approxomately 158".code allows for a span of 148" for this type of lumber. contractor installed a doubled 2x10 beam to compensate but this girder is not supported at theends and the intermediate supports,approximately 90.c. are supported with 4x4 pt post that are set on a 4"piece of block.there are no pier footings as required by code.these would typicallybe 24"x24"x12"poured concrete pier pads.plans werenoted that the joist should be 12"o.c. 2.crawl space vents were cut in between the joist and the band boards instead of into the foundation as is typical.the way the vents are currenty placed proper ventilation of the crawl area and insulation of the floor cannot be accomplished.if vents cannot be lowered with vent wells installed then mechanical ventilation of the crawl area would be recommended(r-311) 3.sill plate,band boards,and floor joist that are supported on the concrete foundation are not pressure treated and are not more than 8"above the finished grade.code requires that sills which rest on exterior concrete or masonary walls and are less than 8"from exsposed ground are to be decay resistant wood or pressure treated materials.(r-309 2) 4. ledger board that is attached to the band boardof the existing dwelling is attached with 3/8 or 1/4 lags at 3 or more o.c..it is questionable that this is adequate.table r-4.402.3a of the code requires a nailing pattern of 16d nail at 32"o.c with two 20d nails at ends.16d nails are at top and bottom.this is required for built up girders.typically tha application of 1/2 bolts or lags at 24" oc staggered at top and bottoms have been accepted.further investigation and comments from a licensed engineer are required on this issue. 5.ceiling joist,and knee wall that carry part of the roof load are bearing on the brick veneer which is not design as a load bearing wall (r-503-4).veneer wall does show some indication ofcracking and movement. 6.ceiling joist in some areas do not have the required the required 1 1/2 bearing and do not have enough nails(r-702.4 and r-402.3a) 7.ceiling joist are 2x6 spf#2 at 16"o.c and span aproximately 16.as per table 7-b of the code these joist are limited to a span of approximately 12.this is a 4 over span in accordance with the code.(r-702.3) 8.on 5/17/96 mr ed schrieber from the permits office,after confering with bill vantz,the plans examiner for the permits office,contacted mr bakerand requested that a design from a structural engineer be submitted for the proposed roof designthis report from whitesell/saville engineering, signed and sealed by mr whitesell,p.e.,was submitted and attaced to the approved building plans that were sent with the permit to the applicant.in reviewing the drawings and then looking at the roof framing it is evident that theplans and proposals submitted by mr whitsel were not followed and because the new roof system was not properly tied in and aligned as instructed on the plans the roof is not in compliance with code and needs to be rereviewed by and engineer. revised plans and repairs done in accordance with the engineers recommendations. 9.mr cory checked to slope of the roof and determined that the slope was less than a 4:12 and closer to a 3.5:12 slope.need to verify if there was a proper application of roofing felts as required for roof slopes less than 4:12 (r-803.3). mr. wantz will contact this office with regard to the court date and will send a copy of his notes at the time of inspection.wkv 3/20/97 schedule court date is 3/24/97.wkv
