Description: Area: lakeside existing improvements: pavement, gutter, curb improvements to be installed: per signed improvement plan cg4781 notes: a) stormwater best management practices (bmps) are required at all times and shall be in accordance with the county's watershed protection ordinance and county director's letter of instruction dli-ld-i. b) obtain an approved traffic control permit prior to start of any work in the county's right of way. please call traffic engineering at (858) 874-4025 for more information. c) compaction shall be achieved by "mechanical compaction" methods and geotechnical compaction reports are required for all compacted sections of the trench, exceptions to compaction testing requirements at the discretion of the director of public works. d) all paved and unpaved paved areas damaged, disturbed, or removed by the work permitted shall be repaired to the satisfaction of dpw's private development construction inspection and road maintenance sections. as shown attached, the only acceptable trench restoration details are regional standard drawing g-24 - type a for asphalt, g-25 - type c for concrete, and g-25 type d for mixed asphalt and concrete sections. in addition, the final surface treatment is to match the existing surface type e) deviations from standard trench restoration requirements must be reviewed and approved by county materials lab prior to start of work. f) if trench exceeds five (5) feet in vertical depth a cal-osha permit is required. please call cal-osha at (619) 767-2280 permittee agrees to indemnify, hold harmless and defend the county and each of its officers and employees from any liability or responsibility for accident, loss or damage to persons or property arising by reason of the work done by permitee, or permittee's agents, employees or representatives.
