Description: Install a kiosk with fuel island,model #6447 by porta-king . . . . . . . 10/19/98 reciecved a faxed letter from porta-kinga technical package is being prepared by there agent for state review and is to be submitted on or before 10/30/98.will follow up on 11/6/98 to see where the review stand and when approval isanticipated.wkv 1/8/98 spoke with mr kohowski with arnold and associates who is the technical consultant ffor "porta king".final coreespondence was sent to the state on 12/18/98 and the building should be in a position for approval.will call and verify with the state on 1/11/98.wkv 1/14/98 called the state and confirmed that the porta king buildings are in the process for approval,however,i spoke with conti patel from state and his comments was that the approval will only be for new buildings that are being produced once approval by the state has been given for thissystem.mr patel said that the sate cannot grant approval on an existing building and that it would be the decision of the local jurisdition to have the building removed or disassembled and field inspected for complinace with the electrical code.spoke with pp and he feels that we should have penske remove the building or verifiy compliance with the building code. wkv 1/27/99 left message for chuck loper to call with regard to the the kiosk.need to have it opened forinspection or removed and replaced with an approved system.wkv 1/28/99 spoke with chuck loper and advised that his two options for this project area as follow: 1. remove the kiosk in question and replace with astate approved kiosk or,2.scedule an inspection with this office office with a technical rep from porta-king so that our office can inspect the electric and confirm compliance with local codes or what needs to be done to correct the violations stress with mr loper the need to get this resolvedand he is to get back with me by no later than 2/2/99 for the inspection.i said that i will follow up with a certified letter .once this is resolved the permit can be released and this project can be closed.wkv 2/2/99 spoke with chuck loper from penske and he has been in contact with porta king and the third party inspector.they are suppose to call me today(the third party)and inform me when someone will be able to go to the site and sign off the building.mr loper said that if they do not call before the end of the day then i am to call him immediately so that he can keep the ball rolling on this project.wkv 2/2/99 spoke with mr kohowski with t r arnold who are the approved 3rd party inspectors for "porta king" and received some copies of correspondence from his company and the state.they will be receiving a systems certification from the state however the state was still looking for accessibility complinace for some of the units. this list was sent to the state and the final addition of "porta king "to the state list will not be issued until the next state update. this kiosk does not fall under accessible units. i spoke with pp on this and since this is less than 20 sq ft and is used for securitiy more than anything else it can be approved as non-accessiblesince it is similar to a 150 sq ft observation/ security area that is exempted for compliance under the state code.i also spoke with the 3rd party engineer and he confirmed that this design is the same that was submitted to the statewith no change. based on the time and the fact that we,as a permit agency,caught this manufacturer and have made them go through the state approval process (a long timely process) and based on the information submitted,our office will approve this unit once the third party agency,has attached their sticker.pp agreed that this was acceptable .i call mr kohowski with t r arnold and faxed direction to the site.he will call me once the sticker has been attached and i will sign off on the permit.wkv 2/17/99 inspected with blaine.stickers on the site from the approved 3rd party inspector verifying compliance with the 1996 electrical code.this is for a diesel dispensing area that is supervised by an attendant.no self service. no gassoline.wkv
