Description: Sr42759 1/27/2016 9:55 pmon 12/29/15, i contacted kain cares on behalf of a resident who resides at 1504 e. 138th avenue unit e, tampa, fl 33613. the tiles in the kitchen were damaged. i spoke with sheila and was advised that a work order would be sent to daniel christian to address the matter quickly. on 1/26/16, i visited the resident and discovered that the tiles had not been replaced and the damage had begun to intensify to the extent that additional cracks in the tile appeared and the floor was becoming unleveled. i contacted kain cares and spoke with jayla ford. i informed jayla ford of my concerns about the cracked tiles and the safety hazard that the unrepaired floor presented for the disabled resident. jayla stated that tiles had been ordered, but she could not tell me specifics on what efforts that kain cares were doing to ensure the safety of the resident until the tiles arrived. jayla became very defensive and argumentative. i asked her to provide me the name of the construction manager, and initially she would not give me the information. i advised her that i would file an official complaint with housing standards and she began accusing me of making threats and being disrespectful towards her. i advised her of the concerns that i had about the tiles not being repaired or replaced. jayla eventually gave me the name of the construction manager, but would not give me any additional information. therefore, i terminated the call. i called back on 1/27/16 to speak with someone else. i spoke with odaly's munoz. again, i expressed concerns about the cracked tiles in the kitchen area and i reported that other areas in the residence also had cracked tiles and unleveled flooring. additionally, i informed odaly's that the lighting in the kitchen was inadequate, the back bedroom had mold, and the bathroom did not have safety rails for the resident who ambulates using a motorized wheel chair. i am very concerned for the disabled resident and the many violations in her apt. e.first name stephanielast name arnoldphone number 813-270-5372mobile number --email address [email protected]

Description: Sr49123: 3/16/20161:09 pm: i moved into this residence on 9/15/15. it took 3 weeks to get a ceiling fan. there is no ventilation in the apartment. the handyman (as the property manager calls him) lives in the other side of the apt. i have repeatedly tripped over all of the debris that is in the yard, outside of my door, etc. on 2/15/16, i notified the property manager of a leak in the kitchen cupboard. she told the handy man who finally showed up on 18th. i wasn't home but was texted by the property manager "he done." i came home later that day to find that there was a leak in the kitchen roof. i called the handyman as instructed by the manager. a few hours later, there were 2 more leaks so i texted (only way she will communicate) the property manager to tell her that the leaks were spreading. i then called the handyman who told me "it's not raining" and "if it was me i would let it leak." i asked him to come. he showed up 9:00 pm and went into the attic(crawlspace) then said he would have to shut water off until the next day when he acquired a part he needed. i told him to come back whenever because i was staying home to clean the mess from the water (for the third time.) he didn't come back until 7:45 sunday night. he went into the attic again, came down and told me "this is only a temporary fix, 6 months maybe" and "the sheet rock is soaking wet." i have been ill since i moved in there. there is a constant 'musty' smell that she told me in the beginning was because it had been closed up. the apartment was filthy and i scrubbed practically every surface before i moved in. the smell came back a couple weeks later. mold is visible in the bathroom-there is no fan in there. only one window can be opened in the whole apartment. i have been advised by my doctor when i told her my symptoms that it was probably from the mold. i texted the property manager that i would give 7 day written notice as specified in the lease unless she wanted to pay for a hotel until it is fixed-also in lease. she ignori
