Description: Entrance,grading,truck rental/repair facility above ground storage tank/pad,freestanding sign, on the premise sign . . . . . 8-4-97 original project set up for grading and entrance was applied for by asad ghattas kld 9/26/97 spoke with tim cambell.the footing inspection was not approved because of the use of shot rock fill in an area of the footing. originally said that this may not be a problem as long as the engineer would verify that the fill was acceptable with the building design.received some faxed reports from the engineer which gave no numbers but confirmed a visual inspection of the material.after some consideration our office spoke with mr cambell again and told him not to pour until the fill material and its quality were confirmed by his engineer.this call was made within 5 minutes of the original call. told mr cambell to cancel the concrete and to forward revised information with the engineers approval.at approximately 3:00 pm mr. prodonovich and myself stopped at the site.2 trucks had already deposited concrete in the footings at the fill areas and a 3rd truck had just pulled into site.spoke with the job super,bryan,and told him that there was to be no concrete poured as per my conversation with mr cambell.bryan said that he had not received the word so i had him call mr cambell and i told him a second time that there was to be no footings poured until the approvals from the engineer were submitted.allowed the contractor to finish with the 3rd truck but said there was to be no more work till certifications were received.wkv 9/9/97 spoke with mr cambell again and reiterated the need for the engineers approval.last conversation as per 2:00 were that he will have another soils engineer at the site to test the material and will give the results to the structural enginer and call us with the approval. told mr cambell again that there is to be no pouring until the soils fill areas have been approved.wkv 10/1/97 copy of soils report was faxed to the office.this was prepared by triad engineering and was sealed by a maryland p.e..called mr cambell and advised that it was ok to proceed and to call for the additional inspections when ready.noted this under inspections and gave copy of the report to the inspector.wkv
