Description: Land development division 5th floor city hall project name: copperleaf eleventh plat general description: misc. information: prc dte cpc dte . public works street lighting services plan review paccar 1301 chouteau traficway. unimproved street driveway culvert sizing. when a new development has perimeter or internal streets which are unimproved we will recommend that the streets be improved if practical. if improvement is impractical and the streets are to remain unimproved we will require that the developer's engineer determine the proper size and cross-section of roadside ditches or swales along the unimproved streets. although not specified it is hereby implied that all driveway culverts and new driveways regardless of the pipe material will be designed to handle the flow of a 10 year storm without over topping the driveway or adjacent street and shall be constructed in conformance with standard d-us. the engineer will also determine the approximate locations of driveways on the unimproved streets and indicate the proper flowline and size of the driveway culverts required. all of these items will be shown on the street or gradings plans. construction of the swales or ditches will be included in either the grading storm sewer or street permit as appropriate. the developer is responsible for maintenance of the ditches until the maintenance bond which was posted for the permit has expired. the developer will be advised of his requirement at the plats review commitee meeting. use the following language: "on an unimproved street include in the grading or street construction plans properly sized driveway culverts and roadside ditches for each lot". in other cases the city engineer's office will provide to the general public the service of sizing unimproved driveway culverts when the property is not part of an overall development of more that one lot and there is no developer or engineer otherwise involved with the project. the following procedure should be followed:. 1. obtain from the applicant or from the permit counter a contour map that shows the entire watershed with the driveway location clearly shown on the map. attached to the map should be the name address and phone number of the applicant and the pipe material to be used. 2. size the culvert to handle the flow from a 10 year storm without over topping the driveway or the adjacent street. the sizing must also be done to accomodate installation of the culvert in accordance with standard d-us. 3. give the counter staff the approved culvert size and then notify the applicant. if the driveway culvert installation is part of a new driveway installation a permit from public works will be required. if the driveway culvert is for a driveway at a location where street flooding is possible where multiple watersheds converge or any other unusual circumstances exist the review engineer shall notify the applicant that an analysis by a professional engineer will be required.

Description: By accepting this permit permittee acknowledges the following: a) that time is of the essence; b) they have satisfied themselves that the owner has provided satisfactory r/w and/or easement(s) in accordance with city code to accommodate the work; c) to limit disturbed area to the permitted acreage of 6.65 acres d) to give city & adjacent residents 48 hrs advance notice prior to starting work; e) to construct the improvements also to install and maintain bmp's as indicated on the permit plan record documents under plan file no. 2013-089a&b&c&d and contract no. 1 with work not starting prior to february 24 2013; (city agrees to early issuance of this permit subject to applicant agreeing to pay a triple permit fee if work is started before the start date indicated herein); f) to complete this work by the permit completion date of march 29 2015 ; g) to coordinate with the work of all other contractors for timely completion; h) to remove/relocate/reconstruct unacceptable work to the satisfaction of the city; i) to remain responsible for the work until permit and bond are released; j) the city will notify the permittee by letter when the maintenance period starts if applicable under the provisions of the bond for this permit; maintenance shall include any and all repairs to damaged components of the facility that may have occurred prior to or during the maintenance period; k) the city will notify the permittee by letter when this permit and the facility constructed under it are final accepted. final acceptance shall not occur until the maintenance period required under the bond has been completed and all maintenance items addressed. l) the permittee has been informed and understands that this permit obligates the permittee (principal under the bond) to provide as-built records as a condition of satisfying the bond performance obligations under this permit even if the design engineer has designated themselves to this obligation on the plans but has not done so. the permittee hereunder will be given 30 days notice to provide said as-builts. m) they have satisfied themselves through missouri one call and field locates the location of all existing utilities that might conflict with the work in this permit prior to starting any work.
