Description: Ele14-02605 may i first state my apology for this error. in regard to the permit above, this work had been completed. attached certification tag 6/27/2014. i soon after fell ill and after length medical tests, etc, i was admitted to an emergency surgery near the expiration of this permit. with health issues and recovery going on i simply failed to submit the paperwork. i rec'd the invoice yesterday evening and have acted promptly to correct this matter. i request you consider my appeal on grounds of medical treatment; consideration of my good standing up to this event and in light that this is my first offense. i will be more timely in submission of my completion cards from now on. thank you. ***see activities
